Enrollment for Petition
Schedule : 6 & 7 January 2020 8am to 5pm only (no extensions)
Venue : GH Campus
Requirements :
- Officially Enrolled Registration Form for 2nd Sem, 2019-2020 (if applicable)
- Load Revision Form – Adding of Petition
- Approved OVPAS Form No. 1 (Request to Open Subject Form)
- Official Receipt
- For Returning Students, accomplished Clearance Form
- For Shifters, accomplished Shifting Form
Steps :
1. Secure a transaction stub from the Guard House.
2. ASSESSMENT – Present OVPAS Form No. 1 at Accounting Office (Room 104, Building A, GH Campus).
4. PAYMENT – Pay assessed fee for Petition at Cashier (Room 201, Building A, GH Campus).
5. REGISTRATION & STAMPING OF ENROLLED – Present OVPAS Form No. 1, Official Receipt, Registration Form and Stub at MIS Center (Room 301, Building B, GH Campus).
Late Enrollment
For all year levels, including returnee, shifters, etc.
Schedule: 6, 7 & 8 January 2020 8 AM to 5 PM only (no more extensions)
Venue : GH Campus
Process : Go to the Accounting Office, then Cashier, then MIS.
Requirements :
- Late Enrollment Fee (please go to the Accounting Office, GH Campus for details)
- Previous Registration Forms
- Advisement Slip, signed by the Program Adviser
- Voucher (For Free Tuition Fee Education Grant Holders-Marikina Residents only)
- Certificate of Scholarship and Financial Aid (if applicable)
- Copy of Gradeslips from previous semesters
- For Returning Students, accomplished Clearance Form
- For Shifters, accomplished Shifting Form
Steps :
1. Secure a transaction stub from the Guard House.
2. ASSESSMENT – Present Advisement Slip at Accounting Office (Room 104, Building A, GH Campus).
3. PAYMENT – Pay assessed fee for Petition at Cashier (Room 201, Building A, GH Campus).
4. CSG Membership
5. SIGNING OF PAYROLL – Sign the Tuition Fee Educational Grant Form and surrender your voucher (for Voucher Holder) at MIS Center (Room 301, Building B, GH Campus).
6. REGISTRATION & STAMPING OF ENROLLED – Present Advisement Slip, Official Receipt and Stub at MIS Center (Room 301, Building B, GH Campus).
Note : Failure to enroll on said schedule means you are no longer interested in pursuing your studies for the 2nd semester, AY 2019-2020. You need to file for a Leave of Absence to be accepted in the succeeding semesters and/or future academic years. Please proceed to the Registrar’s Office for the deadline and how to file for the Leave of Absence.
Furthermore, BENEFITS for tuition-free voucher holders who FAIL to Enroll and/or file for Leave of Absence on said schedule SHALL BE REVOKED.
Load Revision
Schedule : 9, 10, 13 & 14 January 2020, 8 AM to 4 PM only (no more extensions)
Load Revision Forms will be issued at the Office of the University Registrar (O.U.R.). Please check the campus where your program is assigned.
- GH Campus – BSN, BSIT, BSHM, BSHRM, BSTM & BPE
- SSS Campus – BSA, BSBA, BSENTREP, BSCRIM, BAPA, BAC, ABPSYCH, BSPSYCH, BSSW, BSED & BEED
Please take note that the Load Revision Form’s Validity has a due date.
Steps :
1. ADVISING – Once you have received your revision form, proceed to your respective Program Heads/College Deans for advisement of course/s to be dropped or added. Please make sure your load revision form is signed by the head or dean.
For Change of Course/Section, you may proceed to Step 2 (Sectioning). Change of Course/Section may be applied to students with conflict in class schedules or class that was dissolved. Conflict in work schedules must have been considered during the advisement.
2. SECTIONING – Please go to the O.U.R. Campus where the class that you are enrolling is assigned to verify its availability. The class schedule and available slots may be viewed at the portal but please prepare another section in case the class that you are applying is already closed. Class count considered in the portal are Officially Enrolled Students only, while the reservation of slots is being monitored by the O.U.R
3. Secure a transaction stub from the Guard House.
4. ASSESSMENT – Present LRF and Latest Registration Form at Accounting Office (Room 104, Building A, GH Campus).
5. PAYMENT – Cashier (Room 201, Building A, GH Campus)
6. ENLISTMENT & STAMPING OF ENROLLED – Present LRF, Latest Registration Form and Stub at MIS Center (Room 301, Building B, GH Campus).
Note : BENEFITS for tuition-free voucher holders who FAIL to revise their load on said schedule SHALL BE REVOKED.
