Please be advised that the deadline of submission of Completion Form (Form 002A) for Incomplete (INC) grades incurred during the Second Semester, AY 2017-2018 is on April 16, 2019. The said forms should be submitted to the Office of the University Registrar (OUR).
Leave of Absence
All students who are planning to stop or leave out schooling for next semester should file for a Leave of Absence (LOA). A student may file Leave of Absence (LOA) for one semester to be approved by the Dean or Program Head and endorsed to the Registrar Office.
